After some 200 years of public failure, biological psychiatrists or "clinical neuroscientists" still get almost all the money, even though they still can't even reliably diagnose a SINGLE(!) mental disorder with any of their expensive "objective" machines. (And there are HUNDREDS of candidates, e.g. in the DSM-5.)My colleague Laura Batstra, together with some other professors, just published this highly recommendable critical opinion article in the Dutch daily newspaper de Volkskrant.The 20+ million now going to another research project that is doomed to fail (primarily in Amsterdam) are not only an irresponsible waste of taxpayer's money, but also severely harm competing clinical research (that's not funded) and, most importantly, potentially even the lives of millions of patients, some of whom will commit suicide while these "clinical neuroscientists" will continue to look for needles in the haystack that don't even exist (or, in more scholarly language, "the neural mechanisms of mental disorders").I stopped getting angry about such irresponsible funding decisions. But, once in a while, I at least share my (argument- and evidence-based) opinion about it.Biological psychiatry is based on a category mistake. Rond het jaar 2000 begonnen we om de basisprincipes van TCCC in Nederland te introduceren. Sinds 1996 heeft TCCC langzaam en zeker terrein gewonnen. Vele obstakels werden overwonnen door een dedicated groep professionals die tegen de heersende opvattingen over medische zorg in gevaarlijke omstandigheden moesten roeien. Doorslaggevend was daarbij de “evidence based” mindset, gebruik makend van data uit het veld en de diversiteit binnen de groep bestaande uit civiele en militaire specialisten, militaire Medics en andere specialisten, die luisterden naar elkaar en samen tot de zorg verbeterden. In die tijd was dat niet vanzelfsprekend en bestreden militaire specialisten en medische specialisten elkaar vanuit de kennis en ervaringen opgedaan in hun eigen vakgebied/ specifieke werkomgeving. Zo werd een commerciële tourniquet in 2004 nog onnodig geacht omdat “2 driekanten doeken en een stokje” zouden voldoen en kreeg iedere Medic aangeleerd om een trauma slachtoffer met hypovolemische shock eerst 2L kristalloiden te geven, leerden medics om op het slagveld een pneumothorax te diagnosticeren mbv de klassieke criteria, etc. Nu 25 jaar later verschijnt “Tell them yourself” waarin de weg die TCCC heeft afgelegd wordt beschreven. TCCC is militair niet meer weg te denken en heeft ook civiel zijn weg gevonden. Naast de TCCC lesstof en protocollen op www.deployedmedicine.com (of de app) een must have voor iedere tamed trainer/gebruiker en allen die geïnteresseerd zijn in prehospitale zorg in gevaarlijke omstandigheden.This publication is an extraordinary read.
